Follow-Up Support Worker

One Year Contract / Full Time

This person provides mobile case management services for recently homeless clients to enable them to stabilize their housing situations and live independently. The Follow-up Support Worker works as a member of the Albion follow-up team. The successful applicant must have a current valid Ontario driver's licence, use of a vehicle, and a good driving record.

Responsibilities
  • Carry out home visits to clients who are at significant risk of homelessness
  • Provide case management planning, supportive counselling, crisis intervention, and information and referral services
  • Advocate on behalf of clients with landlords, OW, ODSP, and other services
  • Enable clients to establish or maintain links with appropriate services including services related to general health care, addiction treatment, mental health care, identification document replacement, volunteer opportunities, job training, and job placement
  • Provide life skills training in areas including landlord-tenant rights and responsibilities, budgeting, housekeeping, and pet care
  • Transport clients to appointments
Minimum Qualifications
  • Good oral and written communication skills
  • Understanding of issues facing homeless people
  • Skills in assessment, case management, advocacy, and negotiation
  • Personal organizational skills and ability to work as a member of a team
  • Knowledge of community resources and community services
  • Post-secondary education in social services or equivalent combination of knowledge and experience
  • Access to a vehicle that can be used for work

Hours 9:30 am to 5:00pm with some flexibility required

Salary $30,000 per annum
